Breast cancer is the most common cancer among American women after skin cancer.  Treating this disease involves methods such as chemotherapy, radiation therapy, and surgery.  The side effects of some of these methods can cause damage to a survivor’s smile and teeth.

Since 2007, a survivor of breast cancer has received a complimentary smile makeover through this program.  To increase awareness of breast cancer and to restore a survivor’s smile, we are continuing this tradition.  Dr. Craig T. Smith and Highland Heights Dental will donate veneers and/or anterior crowns to a survivor nominee of the Salt Lake Valley in the month of Mother’s Day.  See our gallery of past winners or become a fan on facebook

To nominate your survivor or yourself please submit a close-up photo.  Also include an explanation of your survivor’s story, so we may learn more about you (please limit to one page).  Applications can be dropped off in person to the address below or submitted one of the following ways:

  1.   Emailed to nomination@survivorbrightsmiles.com

  2.   Mailed to 4046 Highland Dr. #115 Salt Lake City, UT  84124

Applications must be received by May 31, 2010.  The winner is selected at random from all the nominations.  Only one nomination per survivor.  This prize is offered only to survivors of breast and ovarian cancer.  Fraudulent submissions will be ignored.  Please continue to donate to breast cancer research at www.komenslc.org and participate in the annual Race for the Cure on May 8th.  We would also like to thank our new 2010 sponsors Julie Williams Photography and Moench Printing.  Julie Williams will provide this years before’s and after’s and a full session of the winner after the bright new smile is complete.
